AI features

Form-generation prompts are sent to OpenAI when you use AI generation. Response summaries require the form owner's confirmation before responses are sent to OpenAI. Do not include sensitive information in prompts or answers.

Email and cookies

Amazon SES sends account verification and recovery email. Essential authentication and anti-forgery cookies protect your account. We do not use advertising cookies in this version.

Responding to a form

The form owner determines which questions to ask and how to use the answers. Contact the form owner about their use of your information. Do not submit passwords, payment-card details or sensitive personal information.

When you begin answering a published form, we record the time and a random attempt ID for completion statistics. Unsubmitted answers are not included. Answers entered in the free template preview are not recorded.

This application is in development. A production retention schedule and account-deletion contact must be configured before public launch.
